Lateral Flow Status Request form
On Wednesday morning you will receive an email asking you to confirm whether you have seen the lateral flow tests for your staff.
The email will show the staff that report to you directly and for who you will be providing results. If these are not accurate please contact a member of HR.
Please click on the button which says Open Update Form (figure 1)
This will take you into a Smartsheet form which will list the first member of staff on your list and a box with today's date, along with a comments box.
Please select the drop down arrow on the date box (figure 2)
You will then see a list of options.
Please select the option that is the most valid
You then have the option of adding any comments
Select the Next Button (figure 3)
This will then load up the input screen for your next member of staff.
Complete this screen in the same manner as before, clicking next to go through and input the relevant information for all of your staff.
When you have completed forms for all of your staff, you will be asked if you are ready to submit your update.
You will have the option of receiving an email confirming your responses, simply check the box.
Select Submit Update (figure 4)
That's it, you are done!
